What is peer review? Meaning, purpose, and the process step by step
Peer review is the evaluation of a manuscript by independent experts before a journal decides whether to publish it. What it means, what it is for, the seven steps, the types, how long it takes, how reliable it is, and how to tell whether an article was peer reviewed. Every number quoted from its source.
What does “peer review” mean?
In scholarly publishing, peer review (also called refereeing) is the process by which a journal asks independent researchers in the same field to evaluate a submitted manuscript and advise the editor. The reviewers, the peers, assess whether the research question is worth answering, whether the design and methods can answer it, whether the statistics are sound, whether the conclusions follow from the results, and whether the report is complete enough for others to judge and reproduce it. They recommend; the editor decides.
A peer-reviewed article is one that passed this process before publication. The label says that qualified readers scrutinised the work and the journal acted on their advice. It does not say the findings are correct, and the evidence on this page shows why that distinction matters.
What is the purpose of peer review in science?
Peer review does two jobs. It screens: editors use reviewers' judgments to decide what a journal publishes, which is why acceptance rates at leading journals are in the single digits. And it improves: reviewers' comments change the manuscript before it is published. Researchers overwhelmingly believe in both functions, and the measured effect on manuscript quality is real, if modest.[1][2]
At Annals of Internal Medicine, 33 of 34 manuscript-quality items improved after peer review and editing; the share of items scored 3 or more rose by 7.3 percentage points from a 75% baseline. [1]
90% of researchers surveyed in 2019 (91% in 2009) said peer review improves the quality of published research, and 85% said there would be no control in scientific communication without it. [2]
Nine out of ten authors believe peer review improved the last paper they published, in a 2009 survey of more than 4,000 researchers. [3]
JAMA received 11,526 manuscripts in 2025 and accepted 10.4% overall and 3.9% of research manuscripts. [6]
What is the peer review process, step by step?
Independent experts evaluate a manuscript's methods and conclusions and advise an editor, who decides whether it is published, usually after revision. It is advisory and iterative, not a vote, and at most journals it happens before publication and behind reviewer anonymity.
What are the types of peer review?
COPE's guideline describes peer review models along six dimensions: timing (pre-publication, post-publication, preprints), identifiability (double-anonymous, single-anonymous, open), mediation (editors mediate, or reviewers and authors interact openly), whether reviews are published, who facilitates the review, and who owns it.[29] The common combinations:
- Single-anonymous. Reviewers know the authors; authors do not know the reviewers. The most common model in medicine.
- Double-anonymous. Neither side is named. Intended to reduce status and affiliation bias, which randomized experiments have measured.
- Open and transparent. Identities are known and, in transparent models, the reviews are published alongside the article.
- Post-publication. Commentary and formal review after the article appears, on preprint servers and some journals.
- Registered reports. The protocol is reviewed and accepted in principle before results exist, removing the incentive to report only positive findings.
The literature contains 22 distinct configurations of "open peer review" across 122 published definitions. [19]
The Cochrane review found no clear-cut evidence that concealing reviewer or author identity affects quality (9 studies), some evidence for checklists (2), and no evidence that reviewer training helps (1). [20]
Open peer review slightly improved the quality of review reports (SMD 0.14) and lowered the odds of rejection (OR 0.56) across randomized trials. [21]
Single-blind reviewers were 1.63 times more likely to recommend papers from famous authors, 1.58 times for top universities, and 2.10 times for top companies, and bid for 22% fewer papers. [22]
The same paper was rejected by 23% of reviewers when a Nobel laureate was the visible author, 48% when anonymized, and 65% when an unknown early-career author was shown. [23]
Reviewers recommended a fabricated trial for publication 97.3% of the time when its result was positive and 80.0% when it showed no difference, and found more errors in the no-difference version, though the methods were identical. [25]
How long does peer review take?
Authors reported an average first-response time of 13 weeks, ranging from 8–9 weeks in medicine and public health to 16–18 weeks in economics, mathematics and the humanities. [17]
About one-third of authors (32%) waited three months or more for a first decision, and 10% waited more than six months; rejected manuscripts took four weeks longer. [17]
Does peer review work, and where does it fail?
Peer review improves manuscripts and blocks obvious problems. It is also inconsistent, misses most inserted errors, is swayed by author status and by positive results, and cannot detect fabricated data from the text alone; most retractions are for misconduct that reading did not catch.[4][5][26]
Across 48 studies and 19,443 manuscripts, agreement between reviewers is low: mean intraclass correlation 0.34 and mean Cohen's kappa 0.17. [4]
BMJ reviewers found an average of 2.58 of nine major errors deliberately inserted into a test paper; training raised this only to about 3. [5]
A 2002 systematic review found editorial peer review "largely untested and its effects are uncertain"; only 19 of 135 identified studies met its criteria. [24]
Of 2,047 retracted biomedical articles, 67.4% were attributable to misconduct (fraud or suspected fraud 43.4%, duplicate publication 14.2%, plagiarism 9.8%) and 21.3% to error. [26]
How can you tell if an article is peer reviewed?
- Read the journal's policy page. Reputable journals state their peer review model on an About or Editorial Policies page, and many now publish decision times and acceptance rates; our journal pages quote them.
- Use a database limiter. Library databases and Ulrichsweb flag journals as peer reviewed or refereed. Google Scholar does not label articles this way.
- Check the article type. Original research, systematic reviews and most review articles are peer reviewed; editorials, news, commentaries and letters usually are not, even in peer-reviewed journals.
- Look at the dates. Peer-reviewed articles typically show received, revised and accepted dates.
What happens after peer review?
Accepted manuscripts move to production. Rejected ones usually go to another journal, and most published papers were accepted where they were first submitted; the ones that were not tend to end up in journals that publish more resubmissions.[27] If you are the author, the preparation that survives review is the same at every journal: the right reporting checklist, statistics that match the design, conclusions that match the results, and references that exist.
